Library Card

For an adult Library card, complete only the adult section, read the policy, then sign the application.

For a Child’s Library card, the parent or guardian fills out both the adult’s name and information plus the child’s name, date of birth, completing all the requested information. Read the “permission area,” and choose from the selections, or leave it blank. When completed please sign.

Cards are free for most Palm Beach County residents, however, applicants residing outside the 33460 zip code may be subject to non-resident fees.

Cards may be picked up at the Library one week after submitting the on-line application. A photo ID with current address is required to pick up the card.
