Prospective patients must register for appointment

PALM SPRINGS – FoundCare, Inc., a nonprofit Federally Qualified Health Center, is partnering with Palm Beach County, the Florida Department of Health and the City of Lake Worth Beach to offer free COVID-19 vaccinations to eligible individuals. By appointment only, FoundCare will be administering 500 doses of Johnson & Johnson’s Janssen COVID-19 Vaccine, which only requires the administration of one shot. 

Vaccine administration will be held from 8:00 a.m. to 12:00 p.m. on Saturday, April 10, 2021, by appointment only. The event will take place at the Sacred Heart Catholic Church located at 402 North M Street in Lake Worth. Those without an appointment will be turned away.

Appointment scheduling will be available online by visiting www.foundcare.org/covid-vaccinations. If you are unable to schedule online, you may also call (561) 967-0365. Online registration is strongly encouraged. 

Five hundred appointment slots will be available. Appointments will be scheduled on a first-come-first-served basis to those who meet the required eligibility guidelines set forth by Governor Ron DeSantis. Those who are eligible to schedule include: 

  • Persons 18 years of age and older;
  • Persons determined to be extremely vulnerable by a physician (form required)
  • Health care workers with direct patient contact
  • Long-term care facility residents and staff.

Those with a scheduled appointment should arrive near their designated appointment time. Those who arrive early will not be seen until their scheduled appointment and will be asked to wait. Those who arrive without a scheduled appointment will be turned away. 

“We are so grateful to Sacred Heart Catholic Church for graciously allowing us to utilize their facilities as well as the City of Lake Worth Beach for assisting us with the preparation,” said Rik Pavlescak, Ph.D., Deputy Chief Executive Officer for FoundCare. “Although we have a limited amount of appointments available, we are doing what we can to provide vaccines to minorities in our community who have been disproportionally affected by COVID-19. These partnerships allow us to do so.”
